Our team is a District Nursing team based in a densely populated, multi-cultural area which is statistically (Office of National statistics) regarded as one of the most deprived areas in London. The referrals received by the District Nursing (DN) team are for adults and older persons who are housebound and require a wide range of nursing inputs. The team provides various forms of nursing care, including continuing care, rehabilitation, chronic disease management etc. also it is also involved in holistic assessments, public health promotion and end of life care.
