In the module we have talked about the relationship between technological complexity and accidents. Write a 3000 word essay discussing whether ‘normal’ accidents as proposed by Perrow can be eliminated by the use of techniques of ‘high reliability organisations’. Would use of these techniques have prevented disasters such as Three Mile Island? Use examples from multiple cases to justify your views.
