An implementation plan helps you know where you are going and an evaluation plan will let you know if you got
there. An implementation plan outlines the goals, measurable outcomes and a time line for the completion of
associated tasks. An evaluation plan allows you to track the progress toward your goals and outcomes. For this
assignment, you develop an implementation plan and evaluation plan for your grant proposal.

An implementation plan requires thoughtful consideration of the people, the programs, and the organization. The
tasks involved in successfully planning for and implementing change must be identified and analyzed for role
assignment and specific responsibilities. A time line is developed to guide the work. Training and development
needs, equipments, and/or support personnel or services are identified and a rollout plan developed that allows
all stakeholders to understand the components of the plan.

An evaluation plan seeks evidence to link processes and results for the organization?s continued development. An
evaluation plan can also communicate progress and results to supporters and a community of stakeholders. Use of a
logic model allows for the development of an evaluation plan to describe how to measure the success of a program.
As you develop your evaluation plan, make certain your outcomes are measurable and are aligned with the
implementation plan.

Use the Learning Resources provided to prepare, including the logic model article for envisioning your
implementation plan, the Program Evaluation website for guidelines for developing an evaluation plan and the
Lumina Foundation website resources as a model for all aspects of writing grant proposals.

Develop an Implementation Plan section that outlines the steps you would take to meet your grant proposal
goals/outcomes. The implementation plan should include:

Inclusion of the section on Involvement of Stakeholders that you developed in Module 4
Then develop an Evaluation Plan identifying how you conduct a formative and summative evaluation of the
initiative. This section should include:

Instruments or assessment processes to measure outcomes
Time lines for the evaluation, including planning, rollout, training, benchmarking and other steps needed to
implement your plan
Use the Learning Resources, particularly the logic model, to help you plan these sections of the grant proposal.
Planning for Innovation in Education-Grant Proposal is due in Module 6.

Develop a grant proposal for implementing a technological innovation to improve processes, practices, or outcomes
of an organization with which you are familiar. The grant proposal includes sections labeled:
• Context
• Need for the Innovation
• The Innovation
• Implementation Plan
o Involvement of Stakeholders
o Sustainability Plan
Evaluation Plan
Appendices (not required)

Address three PhD Learning Outcomes.

innovation and change in education organizations, you will develop a grant proposal for implementing a  technological innovation of your choice to improve processes, practices, or outcomes of an organization with which  you are familiar.
