Select 1-2 of the four assessment documents that BEST FIT the needs of your target child. Do not simply select the shortest or easiest form! To get the most out of this project, you must determine how to best address the needs of your target child and decide which assessment(s) will yield the most meaningful and useful information.
I recommend using 2 of the above forms, one of which being the Support Planning Chart (same one we’ve been using repeatedly during class with application cases).
4. Complete the selected assessment form(s). You may adapt and revise the form(s) as you see fit. For example, if you are unable to complete certain sections (i.e., parent interview) then skip it and simply state why that section wasn’t attempted.

Review and analyze the data you collected with your chosen assessment forms. Draft conclusions and write up your results as follows (total 50 points for this section):
Provide an overview of your target child’s challenging behaviors. Include a brief description of the child (age, sex, background) in a short narrative. (5 points)
Summarize the results of your functional assessment. (20 points)

Draft a behavior intervention plan (20 points). I suggest that you review the handout “APP-B Positive Guidance Plan” to help you with this section.
