Using your research thus far, as well as your logic model, please draft a survey tool of 15 questions, not including your standard demographic questions. Your survey tool should reflect what you have learned about the social problem, how you designed a program through your logic model, and how you will measure program outcomes. More specifically, your tool can be a part of a process evaluation, needs assessment, or outcome study. When drafting your tool, please think about the constructs/objectives you have identified as most important to your program (from your literature review) and design questions that best reflect your theory of change. After you have written your 15 questions, please write 5 demographic questions that will be placed at the end of your tool. Your suvey should have appropriate instructions, skip patterns (if needed), concise language, and clear questions. Think about the order of your questions. Which questions should go first? Make sure your respondent can follow the survey without explanation. Fowler has taught his well. Now, let’s aim for clarity in the questions and ease of response categories. Finally, vary your questions. Think about nominal, ordinal, and interval levels of measurement. Think about what kind of data will help you answer your objective.
2. Consent Form
Most evaluation or and all research projects should protect respondents and provide information about the study, as well as their rights as participants.
