Epidemiology 1793-Philadelphia: The Great Experiment”

OPTIONAL:  For those of you who would like to learn more about yellow fever and discovery of its cause (I recommend this) you may view https://www.youtube.com/watch?v=nwGNz_Oq3qk
Please comment on the items noted below.

1. In the film, the narrators noted many sources the population believed were responsible for initiating the outbreak (the initial cause). Please list several and note why they believed it to be true.

2.  The narrators mentioned several factors that were responsible for the spread of yellow fever in Philadelphia.  Please list several of these factors and indicate why they were thought to propagate the outbreak.

3. Throughout the film, the narrators referred to the number of dead or ill, or sometimes they mentioned the proportion of a population affected.  Given what we’ve learned about ‘counting” cases of disease, determining causes of mortality, and what you’ve read about surveillance, how accurate to you think such data are? What factor may affect the accuracy?

4. The video mentioned remedies that were used to try to treat or prevent yellow fever.  With some, we may laugh and wonder ‘what were they thinking?”  Can you identify remedies we use today despite having little to no evidence that they work to treat or prevent or treat infections?

5.  Yellow fever outbreaks occurred periodically in many US cities and regions around the globe, in fact, they still occur today.  When considering this Philadelphia outbreak and the current Ebola outbreak in West Africa, what similarities in response of the residents did you note? What similarities in public health, medicine, and policy responses did you note?

6. If such an outbreak were to surface today, and we were unaware of the causative agent, do you think we’d be better able to prevent cases and treat the ill?  Why do you think so?   Do you think some factors of life today would exacerbate such an epidemic?  Why?

7.  Did you know about this epidemic prior to the film?  Was what you knew similar to the story presented here? How did viewing the film change your impression of epidemics, and the policy and medical response to health threats?

NOT associated with the video:

8. Consider a health issue of concern or interest to you.  Identify one source of surveillance data for that issue. Please state: the group responsible for the data, the method of data collection, the type of surveillance system,  the definition of the primary health issue, population surveilled, and a few strengths and weaknesses of the system.
