Logic Model-Insulin Administration

Please look at the attached example and do the exact same thing but for Insulin Administration or technique. let us say the diabetic patient on insulin doesn’t know how to inject and we having high readmission rate in this regards. after this logic model is done we want to see a decrease in readmission rate and increase in HCAHPS score regarding understanding insulin.
