Paper instructions:
• Background on the behavior being changed
o In the this section you will need to provide information on what the problem is in your given demographic set

• Model Application
o In this section you will need to identify three studies that have utilized a behavior change theory to change the behavior that you are working on for this class project.
? Describe the findings from the 3 studies that have used the theory to change the behavior
• How were the constructs applied
• What were the results
In this section you will need to include data for peer review journal articles

• Model Application
o In this section you will give a through description of theory that you are using guide your behavior change. Be sure to include citations.
o Then you will describe how you will use the model in your behavior change.

• Behavior Change journal
o This will just be the 6 weeks of journal entries.

• Evaluation
o Description of process, impact, and outcome evaluations and how you would do each for you behavior change
o Then you will imagine that you will be implementing a program similar to what you did for this project. Describe how you would evaluate that program. Include process, impact and outcome evaluation Be sure to include citations.
