Nutrition Diet Planning and Assessment
Each student needs to select a case with whom she will work for at least 2 weeks. It can be any case: weight reduction, weight gain, diabetes, gestational diabetes, cardiovascular, hypertension….
You will need to:
a. Interview
b. Assess
c. Diagnose
d. Plan the diet
e. Monitor
f. Evaluate
By the end of the two weeks, you will need to write a detailed report about your case, including all your assessment sheets, calculations, tables, diet breakdown, menus, also include nutrient breakdown excel sheet screenshot that gives you the grams of each of CHO proteins and fats needed for the patient (i will attach the excel sheet to be filled out)…
You are free to include whatever information you feel will help understand and highlight your intervention.
The following criteria should be presented in your report:
1. Full Assessment (anthropometric measurements, history, laboratory tests, 24 hour recall, physical activity….). Please attach your assessment form.
2. Proper Diagnosis with PES statements
3. Detailed calorie or disease related calculations
4. Clear “Diet Statement” and plan for intervention
5. Your diet (breakdown, calculation sheet and menu plan)
6. Follow up assessment after 1 week with future target for weight, exercise, laboratory tests (if diabetic, hypertensive, hyperlipidemic…)
7. Evaluation of your diet, stating need for change (if applicable)
8. Your new meal plan with the menu (if applicable)
9. Final assessment and recommendations
10. Your feedback and reflection about the experience (strength, difficulties, evaluation of your patient things you will change in the future, things you belief was a success…)
You can include any additional data if necessary.The total grade earned from this project is 10. You will be evaluated on the soundness and the multidisciplinary approach of your reasoning, assessment, intervention, diet planning and reassessment.
