“Breast Engorgement: The Midwives Role in encouraging Successful Breastfeeding and The Benefits of Breastfeeding Establishment. Scenario During a community placement the midwife and the author visited Emily, a 28 year old primipara who was at home, 3 days after giving birth to her first baby, a male infant, vaginally. At the visit, Emily mentioned she had been having difficulty breastfeeding her baby since being discharged from the hospital, suffering from nipple pain especially with latch-on, and that her breasts had become swollen and painful, making it difficult for the baby to attach. She was concerned about the baby not getting enough milk as, despite of being breastfeeding frequently, the baby seemed unsettled and crying all the time, causing her anxiety and stress. Afraid of being starving her baby, and despite her original desire to breastfeed, she had started given formula the night before. On the examination, the midwife noticed the breasts felt hard and tender and the nipples sore. Emily was experiencing breast engorgement. Emily was asked to breastfeed in order to observe the positioning and latching of the baby. Breast support technique was C-hold; however, she was pinching the nipple between the thumb and forefinger and placing the nipple into the babys mouth before any evidence of rooting response, what led to the baby latching just on the nipple and suckling ineffectively.”

This essay focuses on an episode of care observed during my community placement. This episode of care will be critically analysed using up to date references, health care policies and relevant models. The essay will briefly discuss the importance of the role of the midwife in encouraging successful breastfeeding even when difficulties arise. In particular, I have considered talking about breast engorgement during lactation due to its relevance in breastfeeding, being one of the main causes of early weaning, which could be avoided with professional support and effective information.
