Description:
As a part of my Overseas Programme, I have carried out 7-8 holistic assessments on patients with a variety of different presenting conditions. The patients on District Nursing (DN) caseload are adults and older patients who are housebound and come from a wide variety of cultural and ethnic backgrounds. The types of patients I assessed had many different presenting conditions. Most of the referrals made to the DN service are from the GPs and local hospitals.
