Creating and Designing a Care Plan

You are a social worker in the hospital and have a case management role. The social workers are involved in assessment, care planning, monitoring and evaluating the service to provide holistic care.

Your task is to read the following case study and prepare a care plan for John.
You should use the academic literature to inform your practice plan.

The following questions will help you complete the assessment task.

2) How would you go about developing a care plan and who would you consult?

3) What ethical considerations might be there while preparing the plan?

4) Describe key advocacy roles and strategies employed by the social worker in the case scenario. (15%)

5) Evidence of relevant research and use of literature along with sound structure, expression, grammar, spelling and referencing (min: 12 references) (15%)
