SOAP Note and SBAR

Students will complete a written paper about their experience following the simulation. The paper will include 3 items:

1. SOAP note regarding your simulated patient, documenting your time.

2. SBAR on their simulated patient as if you are communicating a change of care to another nurse.

3. A paragraph or two on translating this experience to ‘real life’. Was this simulation effective as a teaching point? Was this experience stressful at all?

Scenario 2: Psychosis
o 35 year old DEF/01F veteran in ED brought in by police
a Patient religiously preoccupied and believes that his family members
are actually demons.
Patient set fire to home in an effort to “cleanse” the evil from his home
a Limited insight into dangerous nature of his behavior
0 Patient doesn’t understand why he was brought to hospital for psych
evaluation
Conflict: Patient with limited insight/ psychosis vs clinician attempts for building
insight and need for psych treatment
Teaching point: Trust building with psychotic patient
Appeal to MD for PRN medication administration
